Job summary

Marion valley post acute in Ohio is seeking a Social Service professional to assist in planning, implementing, and directing our facility’s social service programs. You will work with administration and medical staff to ensure the residents’ psychosocial needs are met and coordinate with community resources.

The role requires a high school diploma (associate or bachelor’s degree preferred) and gives special emphasis to policy development, discharge planning, and maintaining confidential resident

Qualifications

• Must have a high school diploma or equivalent; associate or bachelor’s degree preferred.
• Healthcare experience is preferred.
• Ability to read, analyze, and interpret regulations and procedures.
• Strong written and verbal communication; confidentiality.

Responsibilities

• Assist in planning, development, and evaluation of social service programs.
• Meet with administration and staff to coordinate social services.
• Develop and coordinate department policies and procedures.
• Prepare progress notes on resident social care and treatment plans.
• Coordinate with community agencies to obtain resources.
• Assist with discharge planning and scheduling related services.
• Maintain confidentiality of resident information.
