Job summary

Olympic View Post Acute is seeking a Social Services Coordinator to advocate for residents, guiding them from admission through discharge with compassionate, clear communication. Qualifications

• Bachelor’s degree in social work or related field.
• 1+ year of supervised social work experience in health care.
• Current Social Services certification or licensure.
• Experience documenting in an EMR and basic computer skills.

Responsibilities

• Assist with admission by meeting residents, families, and responsible parties.
• Complete Social Service Assessment, Discharge Planning, and MDS within seven days of admission and keep notes current quarterly.
• Build and document care plans with clear, measurable goals for social needs.
• Guide discharge planning, including placement, home health services, and transportation.
• Assist residents with benefits and financial or legal matters (Medicare, Medicaid, Social Security, VA).
• Keep families informed about health changes and discharge plans and invite participation in planning.
• Support residents in health-care decisions, including advance directives.
• Provide or arrange counseling and support residents who are grieving.
• Coordinate referrals to outside agencies and obtain adaptive equipment and personal items.
• Lead staff and supervise Social Services Assistant(s) to meet individual needs.

Do you want your work to shape how residents and families experience one of the biggest transitions of their lives? Olympic View Post Acute is looking for a Social Services Coordinator to be the trusted advocate for our residents, guiding them and their families from admission through discharge with compassion and clear communication.

What You'll Do

• Support the admission process by meeting with prospective residents, families, and responsible parties

• Complete the Social Service Assessment, Discharge Planning, and MDS sections in our EMR within seven days of admission, and keep progress notes current at least quarterly

• Build and document care plans with clear, measurable goals for each resident's social service needs

• Guide discharge planning, including placement, home health services, and transportation

• Help residents and families with benefits and financial or legal matters, such as Medicare, Medicaid, Social Security, VA benefits, and funeral preplanning referrals

• Keep families informed about changes in health, goals, and discharge plans, and encourage their participation in care planning

• Support residents in making their own health care decisions, including advance directives and who they want involved

• Provide or arrange counseling services and support residents who are grieving

• Coordinate referrals to outside agencies and obtain adaptive equipment, clothing, and personal items

• Explore alternatives to drug therapy and restraints by assessing each resident's needs

• Supervise Social Services Assistant(s) and coach staff on supporting each resident's individual needs and preferences

• Help resolve grievances raised by residents, families, or the ombudsman

• Participate in interdisciplinary care plan conferences, Resident and Family Group meetings, QA programs, and facility committees

• Lead one in-service annually and attend staff meetings and trainings

• Protect resident confidentiality and follow infection control and safety policies

What You'll Need

• Bachelor's degree in social work or a related human services field

• 1+ year of supervised social work experience in a health care setting, working directly with individuals

• Current Social Services certification or licensure as required for this role

• Experience documenting in an electronic medical record, plus basic computer skills (online applications, Word, and Excel)

• Strong communication skills for working with residents, families, staff, and outside agencies

• Ability to read, speak, and understand English
