Job summary

Retirement Housing Foundation seeks a Service Coordinator for Northfield Manor in Northfield, MN to support residents and coordinate services. The role focuses on helping residents access resources, advocate for needs, and promote health and independence.

You will build relationships with community partners, monitor referrals, and develop educational programs while maintaining confidential records and reporting on activities.

Qualifications

• Bachelor's degree preferred in gerontology, social work, psychology, sociology or related field.
• Experience supporting older adults or individuals with disabilities is valued.
• Strong advocacy, communication, and ethical judgment are essential.

Responsibilities

• Assess resident needs and connect residents with social, health, wellness, and community resources.
• Coordinate referrals, follow up on services, and maintain resource directories.
• Prepare and maintain confidential resident records and reports.
• Collaborate with residents, staff, and community partners to promote independence.
