Job summary

West Valley RHF Partners LP operates West Valley Towers, a 97-unit senior residential community in Van Nuys, CA. The Service Coordinator supports resident well-being by identifying needs and coordinating services, building provider relationships, and developing wellness programs.

This full-time role involves advocacy, crisis intervention, and program development, with duties including assessments, referrals, and records management to ensure quality resident care.

Qualifications

• Supportive services knowledge for older adults.
• Advocacy and problem-solving to meet resident needs.
• Commitment to ethical standards and confidentiality.
• Ability to collaborate with residents, staff and partners.
• Ability to work independently with minimal supervision.
• Strong verbal and written communication skills.
• Excellent time management and organization.
• Proficient in Word, Excel, PowerPoint and Outlook.

Responsibilities

• Resident assessment to identify needed services.
• Coordinate access to social, health and wellness resources.
• Develop and sustain health education programs.
• Maintain directory of community resources.
• Advocate for residents and liaise with services.
• Build relationships with community agencies and providers.
• Communicate effectively between residents and service providers.
• Collaborate with management and other departments.
• Provide crisis information, referrals, and support.
• Monitor referrals and ensure follow-up.
• Keep daily and monthly records of interactions.
• Perform other duties to support RHF mission.
