Earlwood Center in Torrance, CA is seeking a Social Services Designee to support residents and families with care plans, placement transitions, and discharge planning under the Social Services Director. You will help residents attain the highest practicable level of functioning in line with federal and state regulations.

Qualifications

• High school diploma or equivalent.
• Ability to work with physically and mentally ill, disabled, and emotionally upset elderly individuals.
• Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care.
• Ability to maintain confidentiality and be discreet.

Responsibilities

• Assist residents and families with adjusting to placement, illness, and the plan of care.
• Help residents attain the highest practicable level of functioning in accordance with regulations and policies.
• Participate in resident and family council meetings as requested.
• Support Admissions and discharge planning services.
