Job summary

Warren Park Health & Living Center is seeking a Social Services Designee/PRSC to join their team. The role provides for person-centered social and emotional needs of skilled nursing residents and promotes growth to each resident’s full potential.

The position involves coordinating with admissions, discharges, and family members, understanding directives and insurance coverage, and ensuring timely discharge planning and documentation. Equal opportunity employer.

Qualifications

• Ability to read, write, and communicate in English.
• Strong verbal and written communication skills.

Responsibilities

• Keeps informed about new and existing community resources.
• Communicates daily with staff regarding admissions and discharges to various units, facilities, hospitals, community and home.
• Discusses choices for advanced directives, financial power of attorney, guardianship, surrogate decision with resident or representative.
• Documents discharge planning as an ongoing review and initiates communication with resident and family to assess discharge needs.
• Ensures home care needs are met by discharge, with arrangements completed 24 hours prior to discharge when date known.
• Completes all required assessments and documentation per resident in a timely manner.
• All other duties as assigned.
