Job summary

Ballardhrc is seeking a Social Services Designee in Seattle to support residents and families as they adjust to placement and the care plan. You will assist with admission, discharge planning, and ensure alignment with federal/state regulations while maintaining confidentiality.

Under the Social Services Director, you will help residents with treatment wishes, support transitions, and participate in family meetings to foster a person-centered care approach.

Qualifications

• Must have high school diploma or equivalent.
• Knowledge of nursing practices, terminology, and regulations for long-term care.
• Ability to organize, prioritize, and meet deadlines.
• Maintain confidentiality and discretion.
• Sufficient computer skills to manage records and documentation.

Responsibilities

• Assist residents and families with placement, illness, and plan of care, aiming for the highest practicable level of functioning.
• Support residents and families with treatment wishes and transitions within the facility.
• Participate in resident and family council meetings and assist with admissions and discharge planning.
