Job summary

Warren Park Health & Living Center seeks a Social Services Designee/PRSC to support skilled nursing residents’ social, emotional, and psychological needs and promote growth to reach their full potential.

Responsibilities include coordinating admissions and discharges, discussing directives with residents and families, and documenting discharge planning with timely approvals. Strong communication and regulatory understanding are required.

Qualifications

• Must read, write, and communicate in English.
• Strong verbal and written communication skills.
• Experience working with geriatric residents preferred.
• Knowledge of Medicare, Medicaid, HMO, and private insurance coverage.
• Familiarity with accrediting organization standards, IDPH regulations, and state/federal regulations related to skilled care facilities.

Responsibilities

• Keeps informed about new and existing community resources.
• Communicates daily with staff regarding admissions and discharges to various units, facilities, hospitals, community and home.
• Demonstrates knowledge of regulations/standards and discusses choices for advanced directives, financial power of attorney, guardianship, surrogate decision.
• Documents discharge planning as an ongoing review. Initiates ongoing communication with the resident and family to assess discharge needs.
• Ensures home care needs are met by the time of discharge, aiming for arrangements completed 24 hours prior to discharge when date is known.
• Completes all required assessments and documentation per resident in a timely manner.
• All other duties as assigned.
