Sagora Senior Living is seeking a Resident Services Coordinator (LPN/LVN) to lead care coordination and caregiver oversight at The Heritage Tomball. You will support the Resident Services Director, manage staffing, develop care plans with physicians, and communicate with families to ensure high-quality senior care.

Qualifications

• Graduate of an accredited school of nursing and hold LPN/LVN license.
• Minimum 1+ year of management experience in Assisted Living or Memory Care.
• Ability to communicate with residents, families, physicians and staff.
• Strong written communication and basic computer skills.
• Available to work evenings and weekends.

Responsibilities

• Assist with recruitment, hiring, training, supervision and evaluation of caregivers to ensure quality services.
• Plan and delegate work assignments and ensure effective scheduling.
• Assist the Resident Services Director with case management for residents.
• Facilitate family involvement and coordinate move-ins/outs for smooth transitions.
• Investigate incidents, identify safety hazards, and implement corrective actions.
• Monitor resident service plans and coordinate wellness activities.
• Ensure regulatory compliance and participate in surveys.
• Perform other duties as assigned.
