Sagora Senior Living is seeking a Resident Services Coordinator (LPN/LVN) to lead staffing, develop care plans, and coordinate with physicians and families for high-quality elder care. You will supervise caregivers, schedule effectively, and support the Resident Services Director in case management across our senior living communities.

The role requires LPN/LVN credentials, 1+ year of management experience in assisted living, and strong communication.

Qualifications

• Must be a graduate of an accredited school of nursing (LPN/LVN).
• Minimum 1 year of management experience in an Assisted Living or Memory Care setting.

Responsibilities

• Assist with recruitment, hiring, training, supervision, and evaluation of caregivers to assure quality services.
• Plan and delegate work assignments, ensuring effective scheduling.
• Assist the Resident Services Director in providing case management for residents.
• Facilitate family involvement with residents and coordinate move-ins/outs.
• Investigate incidents, identify safety hazards, and implement corrective actions.
• Monitor resident service plans and coordinate wellness activities.
• Ensure regulatory compliance and participate in surveys.
• Other duties as assigned.
