Sagora Senior Living, Inc. in Arlington, TX is seeking a Resident Services Coordinator (LPN/LVN) to lead care coordination within our community.

You will work closely with the Resident Services Director to ensure high-quality care for residents and effective communication with their families and medical teams. The role emphasizes staff scheduling, care plan development, and collaboration with physicians and discharge planners.

Competenze

• Graduate of an accredited nursing program as LPN/LVN.
• Minimum 1 year of management experience in an assisted living or memory care community.
• Excellent communication with residents, families, physicians, and staff.
• Strong written communication and computer skills.
• Availability to work evenings and weekends.

Mansioni

• Assist in recruitment, hiring, training, supervision, and evaluation of caregivers.
• Plan and delegate shifts to ensure effective scheduling.
• Support the Resident Services Director with case management for residents.
• Coordinate with families to involve them in resident care.
• Assist in move-in/move-out coordination for smooth transitions.
• Monitor and update resident service plans and safety measures.
• Coordinate wellness activities and ensure regulatory compliance.
