Sagora Senior Living, Inc. is seeking a dedicated Resident Services Coordinator (LPN/LVN) to lead staffing coordination, care plan development, and collaboration with physicians and families across our Tulsa community.

You will support the Resident Services Director, manage caregiver scheduling, and help ensure high-quality care for residents in independent, assisted living, or memory care settings. Join a respected team committed to resident-first care.

Formación

• Must be a graduate of an accredited school of nursing (LPN/LVN).
• 1+ year of management experience in an Assisted Living or Memory Care community in a similar role.
• Ability to communicate effectively with residents, families, physicians, and associates.

Responsabilidades

• Assist with recruitment, hiring, training, supervision, and evaluation of caregivers to assure quality services.
• Plan and delegate work assignments, ensuring effective scheduling.
• Assist the Resident Services Director in case management services to residents.
• Facilitate and encourage family involvement with residents.
• Assist in coordination of resident move-ins and move-outs for smooth transitions.
• Investigate incidents, identify safety hazards, and implement corrective actions promptly.
• Monitor resident service plans and coordinate wellness activities.
• Ensure regulatory compliance and participate in agency surveys.
• Other duties as assigned.
