National Church Residences seeks Service Coordinators for an Atlanta senior apartment community to advocate for residents, coordinate programs, and connect residents with community services.

Qualifications

• Bachelor's degree in social work or related field.
• Two or more years of experience delivering social services to elderly and/or family populations.
• Working knowledge of supportive services and local community resources.
• Demonstrated ability to advocate, organize, problem-solve, and deliver measurable results for residents.

Responsibilities

• Advocates, organizes, problem-solves, and delivers results for residents.
• Coordinate programs and activities for residents.
• Liaise with community agencies, network with service providers, and identify low-cost services.
• Engage residents to identify needs and refer to community agencies.
• Develop a Resource Directory with local providers.
• Sponsor educational events on health care and life skills.
• Provide quality customer service and assist with basic needs.
• Assist residents in obtaining community services like meals, transportation, and day care counseling.
• Monitor feedback from residents and families.
• Manage internal and external relationships.
