Job summary

Catholic Charities and Catholic Charities Community Services in Staten Island, NY seek a Residential Counselor to implement the Community Residence or Permanent Housing Program and help residents acquire life skills for greater independence.

The role involves documenting services, managing crises, coordinating with providers, and supporting daily living tasks, including meals and budgeting. A HS diploma and relevant experience are required.

Qualifications

• High School Diploma required.
• Experience helping individuals with serious mental illness or similar population.
• Ability to manage crises and de-escalate residents.
• CPR/First Aid certification is required or maintained.

Responsibilities

• Provide case management services to all assigned residents.
• Implement individual service plans to assist residents in attaining rehabilitation goals.
• Offer instruction and service delivery in daily living skills such as grooming, meal planning, budgeting, health care, and social activities.
• Document services and interactions in residents’ case records and maintain records.
• Engage residents in discussions about goals and evaluate services.
• Provide ongoing counseling support as needed.
• Draft residents' comprehensive service plans and involve residents in planning.
• Act as liaison for services including scheduling, referrals, and follow-up.
• Manage incidents and minimize risk; notify the manager.
• Integrate care with other agency staff and service providers.
• Attend required meetings and assist with environment maintenance.
• Supervise medication distribution when applicable and escort clients to appointments.
• Plan and carry out recreational activities and follow NYS OMH regulations.
