Job summary

Nick's Health Care Center in Missouri seeks a dedicated Social Services Designee to oversee the Social Services Department, assist with admissions, and support residents with dignity and compassion.

Requirements include 2–5 years in social work (long-term care preferred) and at least 1 year in a healthcare setting. Medicaid/Medicare knowledge is a plus. Benefits include health, dental, disability, vision, EAP, 401(k), life insurance, PTO, and Mayo Clinic resources.

Qualifications

• 2–5 years of Social Work experience, ideally in Long Term Care.
• At least 1 year of supervised social work experience in health care with individuals including those with mental challenges.
• Knowledge about Medicaid and Medicare programs is a plus.

Responsibilities

• Plan, develop, organize, implement, evaluate, and direct the overall operations of the Social Services Department
• Assist with facility admissions
• Manage resident trust accounts and assist with resident shopping
• Schedule resident appointments and coordinate with other departments as needed
• Maintain accurate charts and documentation of resident care
• Engage, converse, and interact with residents to promote a positive environment
• Work primarily during daytime hours, with occasional evening and weekend visits as required
