Normandy Nursing Center in Missouri seeks a compassionate Social Services Assistant to coordinate discharge planning and resident services within our hospital system. You will assess residents, plan care, and collaborate with families and departments to uphold residents' rights and quality of life.

Qualifications

• Bachelor's degree in social work is preferred.
• Current state clinical social work licensure is required.
• Short term rehab experience is preferred.
• Two years of Social Services experience preferred.
• Strong teamwork capabilities and collaboration skills.
• Proficient with email and basic computer use.
• Excellent verbal and written English communication.
• Bilingual skills are a plus.

Responsibilities

• Coordinate social service/discharge planning within the hospital system via effective communication.
• Develop measurable plans to enhance social service delivery.
• Perform psychosocial assessments and reassessments for residents.
• Assist with admissions and ensure proper paperwork completion.
• Engage with residents/families to form care plans respecting rights.
• Collaborate with multiple departments to meet resident needs.
• Promote resident satisfaction and streamline admissions 24/7.
• Maintain relationships with referring community agencies.
