Job summary

Reliant Care Management Company is seeking a Social Services Designee to lead the Social Services Department in a long-term care setting in Missouri. You will oversee admissions, resident trust accounts, appointments, and inter-department coordination while maintaining accurate care documentation.

The role emphasizes empathy, patience, and a strong work ethic, with daytime hours and occasional evenings/weekends.

Qualifications

• 2–5 years of Social Work experience, ideally in Long Term Care setting.
• At least 1 year of supervised social work experience in health care with individuals including those with mental challenges.
• Knowledge about Medicaid and Medicare programs is a plus.
