Job summary

Aston Group is seeking a compassionate Social Services Assistant to support the Director of Social Services in a Skilled Nursing Facility. This Full Time role focuses on resident-centered care, program planning, and collaboration with an interdisciplinary team.

The position requires prior social services experience, preferably in long-term care, and a commitment to regulatory compliance. Bachelor’s degrees in social work or related fields are preferred but not mandatory depending on experience.

Qualifications

• Bachelor’s degree in Social Work preferred; related field considered.
• Previous experience in social services is required.
• Prior long-term care or skilled nursing experience strongly preferred.
• Must meet state licensure or certification requirements.

Responsibilities

• Assist in planning and implementing a comprehensive Social Services program for residents and families.
• Identify social needs and provide counseling, support, and advocacy as needed.
• Maintain accurate documentation related to residents’ social service needs and care plans.
• Collaborate with the Director of Social Services and interdisciplinary team on care coordination.
• Engage with residents and legal representatives facing personal or environmental challenges.
• Support compliance with federal, state, and facility social services regulations.
