Job summary

Rivaya Care affiliated facility in the United States is seeking a dynamic Social Services Assistant to enhance residents' psychosocial well-being and daily living. You will build rapport with caregivers, advocate for residents, coordinate activities with the activities department, and involve families in resident life.

Experience in a healthcare setting is preferred; strong organizational and communication skills are essential in a team-focused environment.

Qualifications

• High School Diploma required.
• At least one (1) year of experience in a healthcare setting.
• Experience working directly with individuals in a skilled nursing facility preferred.
• Good organizational and communication skills.
• Ability to work well in a team environment.
• Passion for helping residents maintain their psychosocial well-being.

Responsibilities

• Develop and maintain a strong working rapport with caregivers to support residents' psychosocial well-being.
• Serve as a resident advocate, ensuring their needs are met at all times.
• Improve the quality and variety of services/activities available to residents.
• Work with the activities department to involve families and guests in resident activities.
• Assist in the discharge planning process, including arranging community resources and alternative placements.
• Participate in the weekend Manager on Duty rotation.
