This full-time Social Services Assistant role focuses on psychosocial well-being, resident advocacy, and coordinating services in a caring, team-based setting.

A high school diploma is required, with at least one year in healthcare; experience in a skilled nursing facility is preferred.

Qualifications

• High School Diploma required.
• At least 1 year of experience in a healthcare setting.
• Experience working with individuals in a skilled nursing facility preferred.
• Good organizational and communication skills; ability to work well in a team.

Responsibilities

• Develop and maintain a strong working rapport with caregivers to support residents' psychosocial well-being.
• Serve as a resident advocate, ensuring needs are met at all times.
• Improve the quality and variety of services/activities available to residents.
• Work with the activities department to involve families and guests in resident activities.
• Assist in the discharge planning process, including arranging community resources and alternative placements.
• Participate in the weekend Manager on Duty rotation.
