Job summary

Extended Care LLC in Des Plaines is seeking a dynamic Social Services Assistant to support residents’ psychosocial well-being and advocate for their needs in daily activities within a skilled nursing setting. You will develop rapport with caregivers, coordinate with activities, and involve families.

Qualifications

• High School Diploma required.
• At least 1 year of experience in a healthcare setting.
• Experience in a skilled nursing facility preferred.
• Good organizational and communication skills.
• Ability to work well in a team environment.
• Passion for helping residents maintain their psychosocial well-being.

Responsibilities

• Develop and maintain a strong working rapport with caregivers to support residents' psychosocial well-being.
• Serve as a resident advocate, ensuring their needs are met at all times.
• Improve the quality and variety of services/activities available to residents.
• Work with the activities department to involve families and guests in resident activities.
• Assist in the discharge planning process, including arranging community resources and alternative placements.
• Participate in the weekend Manager on Duty rotation.
