Job summary

Qualifications

• 2+ years in social services, human services, case management, resident services, or related field preferred.
• Experience connecting residents with community resources.
• Strong communication, advocacy, organization and problem-solving skills.

Responsibilities

• Serve as a resource and advocate for residents, helping identify needs and support services.
• Connect residents with community resources including food, transportation, financial/rental assistance, benefits, healthcare.
• Provide referrals and follow-up to help residents access available services.
• Develop relationships with local social service agencies and community organizations.
• Support older adults and individuals with disabilities in maintaining independence.
• Coordinate educational programs, wellness initiatives, resident activities and community events.
• Maintain confidential documentation of resident services and interactions.
• Collaborate with onsite management and team to address resident needs.
