Job summary

This role emphasizes non-clinical intake, wellness assessments, and follow-up to support aging in place.

The position requires a bachelor's degree in a social services field (or HUD-equivalent experience) and 2–3 years of relevant service delivery.

Qualifications

• Bachelor's degree in social services related field or HUD-required work experience.
• 2–3 years of social service delivery experience.
• Experience with elderly populations or persons with disabilities is preferred.

Responsibilities

• Educate residents and provide referrals to health and community services.
• Coordinate services and monitor follow-up with providers.
• Maintain partnerships with agencies and document resident cases.
• Understand and navigate local, state, and federal benefits and entitlements.
