Qualifications

• Two or more years of experience in social services

Responsibilities

• Coordinate programs and activities for eligible individuals
• Serve as liaison to community agencies and identify services
• Develop and maintain a Resource Directory of providers
• Organize health education events and referrals
• Conduct participant assessments and document in required systems
• Ensure timely follow-up, monitoring and reporting of services
