Job summary

Rocky Mountain Care-Clearfield in Clearfield, UT is seeking a Resident Advocate to support the Social Services Department in planning, coordinating, and evaluating resident social services in alignment with regulatory standards.

You will work with the Social Services Director and care teams to contribute to discharge planning, care plan development, and resident rights advocacy, while documenting actions in medical records and promoting residents’ dignity.

Qualifications

• High school diploma or equivalent is required.
• Ability to read, write, speak, and understand English.
• CPR licensed and trained in first aid.

Responsibilities

• Assist the Social Service Director in planning, organizing, implementing, and evaluating social services programs.
• Meet with administration, medical and nursing staff to plan social services.
• Participate in discharge planning, care plan development, and resident assessments.
• Encourage resident and family participation in care planning and meetings.
• Document social service actions in each resident’s medical record.
• Advocate for residents’ rights and coordinate with families and other departments.
