Job summary

Rocky Mountain Care is seeking a Social Services Director to plan, organize, supervise, and direct all activities of the Social Services Department in line with federal, state, and local standards and the facility’s policies.

The role includes overseeing preadmission services, discharge planning, and integration of social services into the comprehensive care plan, while upholding resident rights and non-pharmacological approaches to care.

Qualifications

• A degree in social work or another human services field to include, but not limited to sociology, gerontology, special education, rehabilitation counseling, or psychology.
• One year of supervised social work experience in a health care setting working directly with individuals is preferred.
• Must have a valid Utah license, working toward license, able to be licensed.

Responsibilities

• Oversee development, implementation, supervision and evaluation of the Social Services Department to meet residents’ well-being and federal/state regulations.
• Complete or direct the social services component of the comprehensive assessment and contribute to the care plan.
• Coordinate preadmission services and interdisciplinary evaluation of residents needs for institutional care.
