Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing social services programs.
• Meet with administration, medical and nursing staff in planning social services.
• Participate in discharge planning, development of care plans and resident assessments.
• Encourage residents or responsible parties to participate in care plan meetings.
• Document social service actions in each resident’s medical record.
• Assist with policy development and annual review to improve quality of care.
• Advocate for residents’ rights and report abuse or neglect as required.
