The advocate assesses psychosocial needs, coordinates services with providers, and supports residents and families by linking to community resources, discharge planning, and staff education.

Qualifications

• Master's degree in gerontology preferred or related field.
• Experience in a social services program for the elderly or related field.

Responsibilities

• Assess and evaluate residents' psychosocial needs and develop goals for necessary services.
• Incorporate social service goals into the resident's plan of care and attend care planning conferences.
• Assist residents and families in utilizing community resources when not provided by the corporation.
• Maintain confidential records and interview residents and families as appropriate.
• Provide supervision for social work student interns and assist in staff development.
• Lead discharge planning as the team lead and participate in the marketing committee.
• Coordinate services with psychiatric providers and OBRA; assist in resident room management.
• Attend Clinical IDT meetings and advocate for resident rights; provide social work consults for DSM diagnoses.
