Job summary

MidPen Housing in Sonoma, California seeks a Services Coordinator II to deliver education, health and wellness, and asset-building programs for residents. The role oversees programs such as After School, Summer, Academic Support, ESL, Health and Education Workshops, and Food Security initiatives, coordinating enrollment and community outreach.

Qualifications

• B.A. or B.S. in Education, Social Work, Sociology, Gerontology, or related field; 1 year of experience in community development/social services.
• Minimum of one year experience in related areas: families, adults, youth in diverse populations; after-school or classroom settings; elder care; program coordination.

Responsibilities

• Delivers onsite programs at assigned locations to meet standards and regulatory expectations.
• Performs outreach by door-to-door, phone, and email to inform residents about services.
• Manages partnerships to support program goals and communicates with supervisor on improvements.
• Provides individualized support and service coordination for residents, including referrals and enrollment.
• Maintains property-level records and tracks service activities regularly.
• Models leadership and fosters collaboration with Property Management and colleagues.
