Job summary

EAH Housing is seeking a Resident Health Services Coordinator to support residents at Bella Terra and Cochrane Village in Morgan Hill, CA. You’ll provide health navigation, non-clinical support, and connect residents to Medi-Cal and community services to promote housing stability and wellbeing.

Required: Bachelor’s in social services or public health, 3–5 years in human services, DMV clearance; bilingual a plus.

Qualifications

• Bachelor’s degree in social services, public health, or related field.
• 3 to 5 years’ experience in human services, community-based work, CHW roles.
• Lived experience or deep familiarity with the communities served.
• Must have valid driver’s license and DMV clearance.
• CHW certification or willingness to obtain preferred.

Responsibilities

• Assists residents in housing stability, health outcomes, and wellbeing.
• Provides health-related support, navigation of healthcare, and social services.
• Connects residents to Medi-Cal benefits and on-site services.
• Coordinates with MCPs, CHW supervisors, and service partners.
• Maintains documentation and participates in program outreach.
