Job summary

Santa Rosa Community Health seeks a dedicated Care Coordinator to assist high-risk, complex clients in accessing health services and social supports. You will coordinate services, review care plans, and advocate for timely primary care appointments to improve health outcomes.

The role requires active engagement with clients, identification of barriers, and collaboration with community partners. Bilingual Spanish/English is required at some locations.

Qualifications

• High School diploma or GED required.
• Community Health Worker Certificate (CHW), Medical Assistant or Certified Nursing Assistant Certificate (CNA) with one year of community health experience, or a minimum of 5 years community health/outreach experience.

Responsibilities

• Review daily referrals; meet with clients to obtain program consent and review care plan.
• Initiate contact and establish working relationship with patients for assistance with health and social services.
• Assist clients in establishing individualized health goals and identifying barriers; provide health education resources.
• Provide advocacy and monitor client progress; coordinate logistics for health goal adherence.
• Assist in timely scheduling of primary care and other medical appointments; may attend appointments with clients.
• Link individuals to application assistance for health coverage programs; perform client benefit enrollment as needed.
• Act as liaison between community-based organizations and patients; maintain relationships with health partners.
