• Participate in on-site review and evaluation of residents' health status, care plans and care provided.
• Maintain ongoing communication with Resident Care Coordinator, Memory Care Coordinator and Administrator.
• Assist with coordinating additional health services residents may need.
• Assist with counseling residents and families as needed.
• Be on call for medical questions and resident emergencies as scheduled.
• Review changes in residents' conditions with the Administrator, Resident Care Coordinator and Memory Care Coordinator.
• Assist in ensuring infection control guidelines are met.
