You will develop individualized care plans, train staff, and engage residents while partnering with families and the Executive Director to ensure a safe, enriching environment for cognition-impaired residents.

Qualifications

• Associate’s or Bachelor’s degree in Nursing, Healthcare Administration, or related field preferred.
• Proficiency with electronic health records (EHR) and basic computer skills.

Responsibilities

• Develop, implement, and evaluate memory care programs that promote cognitive function and social engagement.
• Regularly review and update person-centered care plans with input from families and interdisciplinary teams.
• Provide leadership, coaching, and dementia-focused staff training to ensure high-quality memory care.
• Coordinate care with nursing, physicians, therapists, and other providers to support residents and families.
