Charlestown by Erickson Senior Living seeks a PRN Home Health Physical Therapist to join our campus team. You will implement residents’ care plans, focusing on personal preferences and comfortable, holistic support within a senior living community.

Must have Home Health PT experience and a valid state license. You will collaborate with an interdisciplinary team, provide geriatric therapy, and handle documentation to meet regulatory requirements within our on-site facilities.

Qualifikationen

• Must possess a valid and current Physical Therapy license in the state.
• Experience providing home health PT services for older adults preferred.
• Documentation accuracy to meet reimbursement and regulatory requirements.

• Implement residents’ personal care plans and support physical, emotional, and clinical needs.
• Collaborate with the interdisciplinary clinical team to address residents' care across continuum.
• Provide holistic, resident-centered therapy within the Erickson Senior Living campus.
• Complete documentation to meet reimbursement and regulatory requirements.
