Charlestown by Erickson Senior Living is seeking a PRN Home Health Physical Therapist to join our on-site team. You will work with residents within our campus, implementing personalized care plans and supporting their physical, emotional, and clinical needs.

The role requires Home Health experience and a valid PT license; there is no travel outside the campus, and you will collaborate with an interdisciplinary clinical team in a culture focused on resident well-being.

Formación

• Valid and current Physical Therapy license in the state.
• Degree and training meet Medicare and state requirements.
• Therapeutic experience with older adults preferred.

Responsabilidades

• Implement residents’ personal care plans, focusing on preferences and routines.
• Provide a holistic approach addressing physical, emotional and clinical needs.
• Complete documentation to meet reimbursement and regulatory requirements.
