Charlestown by Erickson Senior Living on our Maryland campus seeks a PRN Home Health Physical Therapist to provide resident-centered care within our continuum of services. You will implement personal care plans, focus on residents' strengths, and collaborate with the interdisciplinary team.

Home Health experience is required. We offer an onsite physician group, access to wellness visits, and a culture of diversity and inclusion.

Qualifikationen

• Must possess a valid and current state Physical Therapy license.
• Therapeutic experience in older adults across multiple diagnostic groups is preferred.
• The ability to complete documentation to meet reimbursement and regulatory requirements.

• Deliver resident-centered home health physical therapy within the Charlestown campus.
• Implement residents’ personal care plans and support mobility.
• Collaborate with the interdisciplinary clinical team to address residents’ needs.
