Job summary

Charlestown by Erickson Senior Living is seeking a PRN Home Health Physical Therapist to join our campus-based team. All residents live on our award-winning campus, enabling ongoing therapy that aligns with each resident’s goals and routines.

This role requires Home Health experience and a valid Physical Therapy license. You will implement care plans, collaborate with the interdisciplinary team, and manage documentation for Medicare and state requirements. No travel is involved on campus.

Qualifications

• Must hold a valid and current state Physical Therapy license.
• Experience with Medicare/state documentation requirements.
• Therapeutic experience with older adults preferred.

Responsibilities

• Provide home health physical therapy on campus to Erickson residents.
• Implement residents' personal care and therapy plans.
• Collaborate with the interdisciplinary clinical team to address needs.
• Complete documentation to meet reimbursement and regulatory requirements.
