Charlestown by Erickson Senior Living is hiring a resident-centered PRN Home Health Physical Therapist to join our team. All residents live within our award-winning campus, enabling you to support their journey across the continuum of care with a holistic approach.

Experience in home health is required, and candidates must hold a current state Physical Therapy license. Onsite medical centers and a supportive interdisciplinary team are provided.

Qualifications

• Must possess a valid and current state Physical Therapy license.
• The ability to complete documentation to meet reimbursement and regulatory requirements.
• Therapeutic experience in older adults across multiple diagnostic groups is preferred.

Responsibilities

• Provide holistic physical therapy to residents on campus.
• Implement residents' personal care plans focusing on their preferences and routines.
• Collaborate with the interdisciplinary clinical team to address residents' needs across care settings.
• Maintain documentation to meet reimbursement and regulatory requirements.
