Job summary

Charlestown by Erickson Senior Living on a scenic Maryland campus is seeking a PRN Home Health Physical Therapist to join our team. You will deliver resident-centered therapy, implement personal care plans, and collaborate with an interdisciplinary team to support physical, emotional, and clinical well-being.

The role requires Home Health experience, a valid state PT license, and a focus on aging populations.

Qualifications

• Must possess a valid and current state Physical Therapy license.
• Ability to complete documentation to meet reimbursement and regulatory requirements.
• Therapeutic experience in older adults across multiple diagnostic groups is preferred.

Responsibilities

• Implement residents' personal care plans with a focus on individual strengths, preferences, and routines.
• Provide holistic physical therapy within the on-site Erickson Senior Living campus.
• Collaborate with interdisciplinary team to address residents' clinical needs and support well-being.
