Job summary

Rivaya of Des Plaines is seeking a dynamic and compassionate Activity Aide to elevate daily living for residents in an Extended Care setting. You will lead group and one-on-one activities, ensuring engagement and safety while documenting care plans and resident records.

Qualifications

• 1 - 2 years of experience in a social or recreational program in a healthcare setting (preferred).

Responsibilities

• Facilitate ongoing group and individual activity programs based on residents' needs, interests, and abilities, as identified in assessments and care plans.
• Complete required record-keeping upon resident admission, discharge, and transfer; chart and document per facility policy.
• Assist in creating the activity calendar and coordinating events. Follow and maintain established policies and procedures.
• Encourage resident participation in Resident Council and schedule monthly meetings.
