Qualifications

Responsibilities

• Review and update department policies and procedures annually.
• Assist in identifying residents’ medically related social and emotional needs.
• Assist in discharge planning and coordinating community resources.
• Maintain regular progress notes indicating responses to treatment plans.
• Provide information on Medicare/Medicaid and financial assistance options.
