Job summary

PACS Group is recruiting a Social Worker for a full-time role at a Mesa Hills Post Acute facility. The candidate will assist in planning and directing social services, coordinating policies, and supporting residents and families with psychosocial needs.

A BS in Social Work or related field with supervised experience is required, with licensure as applicable. The role involves discharge planning, collaboration with medical staff, and ensuring program compliance with federal and state guidelines,

Qualifications

• Bachelor’s Degree in Social Work (BSW) required or a Bachelor’s Degree in a human services field with one year of supervised social work experience in a health care setting.
• Minimum of one year of supervised social work experience in hospital, nursing facility, hospice, or related program. Long-term care or skilled nursing facility experience preferred.
• Social work licensure (LSW, LMSW, LCSW, or state equivalent) if required by state law; must meet state/federal qualification standards.
• HIPAA training and resident rights and abuse prevention trainings may be required.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing social service programs in accordance with standards.
• Meet with administration and staff to plan social services and coordinate policies and procedures.
• Assist in discharge planning, resource coordination, and resident/family counseling to meet medically related social needs.
