Job summary

Centennial Post Acute is seeking a compassionate and experienced Social Services Director to lead our social services department and improve residents’ emotional and social well-being. You will develop care plans, advocate for residents, and coordinate with providers, families, and resources to ensure comprehensive, compliant care.

The role requires a Bachelor’s degree in Social Work, prior leadership experience in social services within healthcare, and licensure eligibility.

Qualifications

• Bachelor’s degree in Social Work or related field required.
• Licensed Social Worker in Alaska or eligibility to obtain licensure is required.
• Demonstrated compassionate leadership and resident-centered care.
• Excellent organizational skills and ability to manage documentation and compliance.

Responsibilities

• Develop and oversee individualized social service care plans for residents.
• Act as an advocate for residents, ensuring needs and concerns are addressed and respected.
• Coordinate with healthcare providers, family members, and community resources to support care.
• Ensure compliance with state and federal regulations and maintain accurate documentation.
• Lead and mentor the social services team to promote a collaborative care environment.
