Job summary

Pikes Peak Post Acute is seeking a social services professional to assist in planning, developing, organizing, implementing, evaluating, and directing our facility’s social service programs. You will work with administration, medical and nursing staff to coordinate care and ensure residents receive comprehensive psychosocial support.

Qualifications

  • Must possess, as a minimum, a high school diploma or its equivalent.
  • Associate or Bachelor’s Degree preferred but not required.
  • Ability to read, analyze, and interpret general business materials.

Responsibilities

  • Assist in planning, developing, organizing, implementing, evaluating and directing social service programs.
  • Meet with administration, medical and nursing staff to plan social services.
  • Develop, administer and coordinate department policies and procedures.
  • Review policies annually and participate in revisions.
  • Provide information on Medicare/Medicaid and financial assistance options.
  • Record and maintain social service progress notes and treatment plans.
  • Coordinate social service activities with other departments.
  • Assist with discharge planning and community resources.
  • Assist in developing the department budget and training programs.
  • Maintain confidentiality of resident health information.
