Qualifications

• Bachelor's degree in social work or related human-services major.
• One year of healthcare setting experience with direct client interaction.
• Strong communication and writing abilities; ability to work in a team.

Responsibilities

• Provide social services to residents and families under supervision.
• Meet new residents on admission and complete initial psychosocial assessment.
• Coordinate with Medicaid, Medicare, SSI and other community resources.
• Arrange items for residents and schedule visits to dental, eye, podiatrist and audiologist.
• Assist residents with gathering documents and provide support during times of loss.
