Job summary

Reliant Care Management Compan is seeking a Social Services Director to oversee social services for residents and families within our facility. The role includes meeting new residents on admission, completing Initial Psychosocial Assessments, and coordinating follow-ups with residents and families.

You will arrange needed items, schedule visits to healthcare professionals, provide support during times of loss, and help residents gather documents for Medicaid, Medicare, SSI, and other services,

Qualifications

• Bachelor's degree in social work or related field is required.
• Experience interacting with residents and families; ability to complete psychosocial assessments.
• Strong communication and teamwork skills are essential for coordinating care.

Responsibilities

• Provide social services to residents and families under the direction of the social services director.
• Conduct admission visits and initial psychosocial assessments.
• Arrange items needed for residents and coordinate healthcare visits (dental, eye, podiatrist, audiologist).
• Support residents during times of loss and assist with obtaining supporting documents for Medicaid, Medicare, SSI, etc.
• Perform other duties as assigned by the Social Services Director.
