Job summary

Reliant Care Management Company is seeking an experienced Social Service Director to lead resident support and coordinate services with compassion and competence.

You will conduct initial psychosocial assessments, monitor residents' well-being, and support families through care planning and community resources.

A bachelor’s degree in social work or related field, plus healthcare experience, is required along with strong communication and teamwork skills.

Qualifications

• Bachelor’s degree requirement in social work or a related human-services major.
• One year of healthcare setting experience as a supervised social worker.
• Ability to communicate effectively with residents and families.

Responsibilities

• Provide social services to residents and families under the direction of the Social Services Director.
• Meet new residents upon admission and complete initial psychosocial assessments.
• Visit residents to follow up on concerns and assist with coordinating supports.
• Arrange to purchase needed items for residents and ensure adequate clothing.
• Coordinate visits to healthcare professionals (dental, eye, podiatrist, audiologist) as needed.
• Assist residents in gathering documents for Medicaid, Medicare, SSI, etc.
• Perform other duties as directed by the Social Services Director.
