Atrium Centers The Timbers of Cass County seeks a Social Services professional to support residents in a 100% employee-owned environment. You will assist with social service programs, documentation, and care planning, collaborating with families and interdisciplinary teams to deliver compassionate, compliant care.

Applicants with an Associate’s degree preferred and experience in long-term care are encouraged.

Qualifications

• Associate’s Degree preferred in human services or related field.
• Experience in skilled nursing or long-term care preferred.
• Strong written and verbal communication with residents and teams.
• Ability to assist with documentation, assessments, and care planning.
• Compassionate, resident-focused approach.
• Organized and detail-oriented with multitasking ability.
• Knowledge of resident rights and discharge planning preferred.
• Positive attitude and reliable attendance.

Responsibilities

• Assist the Social Services Director in implementing social service programs in line with guidelines.
• Support identifying and documenting residents’ social, emotional, and psychological needs.
• Provide service coordination and communication with residents and families.
• Participate in care planning and interdisciplinary team meetings.
• Maintain accurate, timely documentation to support compliance.
