Job summary

Douglas Manor, Windham CT (part of the Ryders Health family), is seeking a full-time Social Worker to support resident well-being, coordinate services, and advocate for resident rights. You will work with residents, families, and a multidisciplinary team to ensure holistic, person-centered care, spanning assessment, counseling, and care transition planning.

Qualifications

• BSW required; MSW preferred.
• CT LMSW (or eligibility) required; LCSW preferred.
• 1–2 years in social work, preferably with older adults or in long-term care.
• Knowledge of state/federal long-term care regulations and resident rights.
• Strong communication and collaboration with residents, families, and care teams.
• Crisis intervention, counseling, and supportive services experience.

Responsibilities

• Perform psychosocial assessments and create individual care plans.
• Provide counseling to residents and families for adjustment, grief, and stress.
• Advocate for residents’ rights and preferences in care planning.
• Coordinate communication among residents, families, and interdisciplinary teams.
• Lead discharge planning with community resources and home-care options.
• Assist with accessing community services and benefits as appropriate.
• Document assessments, interventions, and plans per facility policies.
• Serve as resource to staff on psychosocial needs and staff education.
• Provide crisis intervention during emergencies affecting residents.
