Qualifications

• Bachelor’s degree in Social Work (BSW) from an accredited program required; MSW preferred.
• Active Connecticut social work license (e.g., LMSW) or eligibility to obtain state licensure; LCSW preferred.
• Minimum of 1–2 years of professional social work experience, preferably with older adults or in long-term care or healthcare settings.
• Experience in psychosocial assessment, care coordination, discharge planning, and case management.
• Knowledge of state and federal long-term care regulations and resident rights.
• Strong interpersonal and communication skills; ability to work collaboratively with residents, families, and multidisciplinary care teams.

Responsibilities

• Conduct comprehensive psychosocial assessments of residents to identify needs and develop care plans.
• Provide counseling and support to residents and families to address adjustment, grief, and stress.
• Advocate for residents’ rights and needs to ensure quality of life.
• Facilitate communication among residents, families, and care teams.
• Coordinate discharge planning and connect residents with community resources.
• Assist residents and families in accessing community services and benefits.
• Participate in care conferences to integrate psychosocial goals into planning.
• Maintain accurate documentation of assessments, interventions, and plans.
